Plan reviews go a lot more smoothly when you’re not scrambling the week before. Here’s the checklist we walk participants through, spread across the four weeks leading up to your review.

Four weeks out: Start a simple list of what’s working and what isn’t in your current plan. Small frustrations are easy to forget by the time your review comes around, so jot them down as they happen.

Three weeks out: Gather any supporting evidence — reports from allied health providers, changes in your circumstances, or anything that shows why a support is or isn’t meeting your goals. If you need a new letter from a GP or specialist, this is the week to ask, since these can take time to come through.

Two weeks out: Think about your goals for the next 12 months, not just what you need right now. Plans work best when they’re built around where you’re heading, not just where you are.

One week out: Have a run-through with your support coordinator or a trusted family member. Talking it out loud usually surfaces a question or two you hadn’t thought of.
